In your case, the cause maybe by php version issue. You should check your PHP version, my theme supports for PHP 5.6.x or 7.1.x. If it does not work, please provide your FTP account again.My coders will check and fix asap.

Yep, I mean in demo, we just have popup to inform to the buyer that the product has been added to cart. It does not have same form as yours.
Even in homepage or in single product page, you have another popup (Go to cart..), but demo site does not have.
