Both my issues are resolved but now on my-account page I can't see register form.

The cause maybe by you have not enable it. Please login the Dashboard>>Woocommerce>>Settings>>Accounts &Privacy Tab and tick choose the Allow customers to create an account on the "My account" page option as the attachment screenshot below. Please connect to us again if it does not work.

The option is on already, this is cuz of theme because u can see or option in the screenshot I shared above.
