    Intake Form or Questionnaire

    Hello!! I would love to know how to implement an intake form or questionnaire form as our customers purchase and log in to fill information out. Something similar to the contact form, but used when they log-in. Is that possible? If so, could you direct me how to do it on our site or what plug-in I will need to do this?

    Hello!
    On our website - there is a gear on the right corner with My Account/Log In/Register.

    Where can I edit the information under "My Account" - once a customer is logged in - what to show and what not to show.
    Currently, under that (in a gray box) are a list of things. We want to create a question/answer form - as a topic in that list.
    Please let me know if it is easier to speak to you regarding this.

    Honestly, if there is a way to add more in-take boxes on the checkout page, that would be extremely helpful!! On the "Checkout" Page, we would like to include a few additional questions for them to fill in. Where can I do this?
