Thank you for responding so promptly.

I have attached two (2) screenshots: one of my current contact form and a second of the demo default contact form.
As I appreciate your help, what I was looking to correct was the positioning of the fields. If you see my screenshots, you will understand.

Is it possible to share a copy of the code (possibly with div attributes) for the layout as seen in the DEMO02?
