impossible to upgrade the woocommerce theme file and my plugins EXTRA FEES doesn't work with your theme

Hi, I'm Francesca from italy, you old customer.
last week I have extend support, so I ask for your assistance.
I have 2 issue:
FIRTS: I need to update the woocommerce kute boutique files. I downloaded the last update in themeforest website, but inside the foldes there aren't all of the files that I need, for example in "kute-boutique/woocommerce/checkout/" there is only "form-checkout.php" file.
Also, this file "/wp-content/themes/kute-boutique/woocommerce/checkout/form-checkout.php" generate a double "woocommerce checkout review order".
So how can I upgrade all of my obsolete files?
I waiting for your support.
SECOND: when I purchased thi template in ottobre 2016, I needed to add 5€ of extra fees for "cash on delivery" payment method. For to do this I buyed a woocommerce plugins "Payment Gateway Based Fees", but it doesn't worked with your theme. So in October 2016 your team support me and discovered an hooks and updated the layout, direclty in my web site and resolved the issue, whitout tell me in which file they have intervened (you can reread our Skype conversation beetwen Jim Brown an me - francesca.adv). But after the wordpress and woocommerce upgrade this plugin doens't work anymore. So I need your support again.
Please help me as soon as is possible.
