vouchers form problem in cart check out

Hello!
There is one problem in my cart check out only when I am using vouchers codes. Two forms instead of one appear in my cart. I would like to fix that. I add one screenshot. Is there any options in the prestashop admin that I can change in order to fix this? Or any module that I should desactivate? Anything that doesn't involve touching the code?
Please I need your answer, I dont want to modify the code... but if I have to... I can do it myself. I want a solution that doesn't involve changing the code.
