Contact page

Hi, I have to realize the contact page. I have two possibilities:
1) Use your page already done, but I want to remove the google map. I tried to remove the coordinates but I see a blank rectangle disappears map
2) Create a new CMS page, but do not know how to insert the contact form. I would also add the enrollment form to the newsletter. In parallax template appears as a pop-up homepage. I turned off because I do not want to appear in homeapge. How can I add it below the contact form?
