Header missing on pages in Boutique theme

Hello,
On my website, the header is missing from my pages. The ONLY pages the header is showing is on the Home page and the Services page. All other pages, the header is missing. I've attempted to create new pages, no luck. I've changed theme settings and tried to manually choose which header to use, no luck. I've even duplicated the page with the headers that are showing but the header doesn't show on the duplicated page. I've cleared my WP cache and purged data, no luck.
I'm out of options and need assistance.
