Problems with Product Page and Permalinks

Hello,

Thank you for you template, it is great.

I have the following problems:

1. my product page do not work. The product permalink i want to be the following: mywebsites.com/category/subcategory/product

2. on the shop page-> on the left sidebar the widget 'Woocommerce Product Category' doubles the permalinks (Print Screen 2)

3. on the shop page the page banners do not appears even if i insert an image

Please help me as soon as posible.
