Missing fields adding an adress

Good afternoon,

I hope you can help me with this problem.

The input boxes of the postal code an the NIF fields doesn't show when a customer need to register on our website. More specifically when adding an adress.
This problem only occurs on the smartphone platform. In a PC works perfectly.

Given the importance of the mobile really hope you can help me.
